Is your organization a Government Agency, Public School, 501(c)(3) or have you recently applied for 501(c)(3) status?

Is your organization located in Nebraska? If not, have you received support in the past from The Sherwood Foundation?

Do you have a board-approved policy stating that your organization will not discriminate in its employment practices, volunteer opportunities, or the delivery of programs or services, on the basis of race, religion, gender/gender identity, national origin, age, medical condition, handicap, veteran status, marital status, or sexual orientation? Policy must explicitly cover each of these areas.
